Program Manager

Function: To provide supervision of adult day habilitation programming within the Medicaid community care and support programs.

Responsibilities:

  • Provides leadership, supervision, professionalism, support and continued development of the day habilitation program(s);
  • Oversee the day-to-day operations of the day habilitation program, which can include developing program curriculum, ensuring delivery of service, supervising employees and assuring certification and other compliance requirements are met;
  • Participates in the continued development and implementation of the day habilitation program policies and procedures;
  • Develops and maintains key working relationships with public and community agencies that support the goals of the community-centered and community based practice;
  • Participates in the hiring, training and disciplining of program staff;
  • Ensures team members have met and maintain the current mandatory training requirement pre- and post-hire;
  • Trains support coaches on their day-to-day responsibilities, as well as the agency and program policies and procedures;
  • Ensures all direct reports are proficient in implementation of programming in accordance with all plans and protocols, including, but not limited to, feeding and medication protocols and other health and safety related protocols and procedures;
  • Participates in the outreach/intake process and enrollment of individuals;
  • Assists in the development and modification of individual service plans (ISP);
  • Participates in interdisciplinary team meetings and other meetings related to participants services;
  • Assures all program specific goals and objectives are established, appropriate, and the outcomes are met;
  • Ensures all day habilitation program spaces are maintained in excellent repair and meet the highest of standards of cleanliness and safety at all times;
  • Responsible for regular audits of participant records, logs and required documentation;
  • Responsible for ensuring program participates are actively engaged in groups and activities appropriate for them, and for ensuring the participants’ needs are met;
  • Monitors staffing coverage to ensure participants’ safety and to maintain the appropriate staffing and supervision ratios required by regulation and ISPs;
  • Ensures all applicable documentation is completed accurately and within established timelines;
  • Models the program expectations and professionalism outlined by the agency;
  • Represents the agency’s mission and purpose to interested individuals, staff and groups;
  • Participates in maintaining the agency’s overall commitment to quality services and supports;
  • Other duties as assigned by the director, community supports, disability support services

Qualifications/Requirements:

  • Must be at least 18 years of age;
  • Bachelor’s level professional with a combination of three years of administrative and program development experience preferred. HS diploma with at least five years administrative experience with intellectual/developmental disabilities. An equivalent combination of experience and education will be considered;
  • Preferred: Medicaid/Fee-for-Service experience and electronic health record;
  • Knowledge of and sensitivity to cultural diversity issues;
  • Ability to work effectively with clients, including intervention and relationship building skills.
  • Ability to work effectively with people of diverse cultural/economic/racial/religious backgrounds and sexual orientation;
  • Team-oriented with strong organizational and collaborative skills and the ability to multi-task;
  • Adapts well to changes in assignment and priorities with the ability to respond effectively to new information, changing conditions, and unexpected obstacles;
  • Excellent communication skills, both oral and written;
  • Must cooperate with the licensee and Department of Human Services (DHS) staff in any inspection or investigation;
  • Strong computer skills necessary:
  • Valid driver’s in the state of residence required and a good driving history; and
