Moab Regional Hospital is looking to fill one full-time position for an experienced Clinical Care RN to be responsible for directing patients to the appropriate route to receive care in the clinical setting. This person will be part of a team that focuses on patient navigation, assists with chronic disease case management, and provides patient education and assessment.

Responsibilities

Clinical Care Navigator Responsibilities:

  • Assist patients telephonically, using email, or in person to help patients understand available options to be seen by a provider.
  • Focuses on enhancing positive patient experience through strong customer service and communication skills.
  • Provide telephone education and advice when necessary and appropriate.
  • Deliver education and referrals if needed.
  • Take patients vitals such as blood pressure, temperature, and weight.

Chronic Care Management and Annual Wellness Visit Responsibilities:

  • Retrieve and review hospital and/or post-acute care records and gain an understanding of patient health issues.
  • Enroll patients into care management services.
  • Provide comprehensive condition-specific self-management training, education, and coaching including diabetic education.
  • Responsible for identifying any change in patient status and notifying primary care physician of change statuses.
  • Provide telephone education and advice when necessary and appropriate.
  • Conduct Health Risk Assessment and other screenings for Medicare patients.
  • Take patients vitals such as blood pressure, temperature, and weight.
  • Deliver education and referrals if needed.
  • Participate in the development of the plan of care with other team members and safely implement the plan.

Requirements

  • Current, unencumbered Utah or compact state RN license
  • 2 year’s RN experience.
  • Graduate of an accredited school of nursing.
  • Excellent customer service and communication skills including oral, written, and listening with providers, co-workers, patients, and patient families.
  • Knowledge of infection control principles, patient safety techniques, and prevention of adverse reactions to medications/treatments.
  • Demonstrated critical thinking skills and ability to maintain such in situations with rapidly changing circumstances.
  • High level of self-motivation and able to work well both independently and as part of a small team.
  • Comprehensive knowledge of a variety of pharmacological agents used in patient treatment, including dosage calculations.
  • Knowledge of computer systems and proficiency in use of basic word processing, data entry and electronic medical records.
  • High level of comfort with patient triage. Able to perform independently.
  • Schedule flexibility, including willingness and ability to regularly work evenings and occasional weekends and holidays.
  • Must be reliable, punctual, and emotionally mature.
  • Ability to work in a confidential manner.
  • Must have a positive attitude. Knowledge of the principles and skills needed for practical nursing to provide patient care and treatment.
  • Knowledge of common safety hazards and precautions to establish a safe work environment.
  • Requires adherence to all policies and procedures, including but not limited to standards for safety, attendance, punctuality and personal appearance. As well as the MRH Employee Expectations as outlined in the employee handbook.
  • Must be flexible and willing to accept other responsibilities that may be assigned.
